The Event form makes it easy to customize the content of multiple fields, checkboxes, radio buttons, and drop-down menus with options so you can gather information from possible attendees quickly and easily.

Included in this template are the following fields:
– Title
– Name
– Phone number
– Email address (marked with a star to indicate it’s required)
– Questions with choices listed in checkboxes (for selecting single or multiple options) and radio buttons (for selecting a single option).
– Menu with options (for selecting a single option)
– Message box (for open questions that the attendee may have)

– Footer text (for adding text to the bottom of form)
